You will need to the following items to enroll your child:

Enrollment Application Check LIst for Kindergarten:

• Student's birth certificate
• Student's immunization records
• Proof of Address (phone, gas or electirc bill or rent, lease, or mortage agreement)
• Student's social security card
• Photo ID of parent or guardian
• Proof of guardianship, if applicable
• All court documents about child custody, if applicable *(Notarized statements are not acceptable)

Transferring from another Cincinnati Public School

• Proof of Address (phone, gas or electirc bill or rent, lease, or mortage agreement)
• Photo ID of parent or guardian
• Proof of guardianship, if applicable
